CPT 99283: ER visit, level 3. What Medicare pays and what to ask
99283 is the middle emergency department visit level. Find out what it covers, what Medicare pays nationally, and what to check on your bill.
What this code means
CPT 99283 describes an emergency visit with low medical decision making, such as a problem that needs an exam and a limited work-up.
The two lower levels are 99281 and 99282. The two higher ones are 99284 and 99285.
Why the bill can be much higher than the Medicare rate
Even at a lower level, the hospital charge can be a multiple of the Medicare rate once a facility fee is added.
Costs on the same bill from labs, imaging and medications are billed separately and often add up to more than the visit itself.
| If a line is billed at | It would be |
|---|---|
| 5x the Medicare rate | $347.35 |
| 10x the Medicare rate | $694.70 |
| 20x the Medicare rate | $1,389.40 |
The Medicare rate is a published benchmark, not a legal ceiling on what a provider may charge. Our free scan flags a line as worth a closer look when it is billed at more than 10 times the Medicare rate.
